Investigation – Cook County Health Data Breach

Wolf Haldenstein is investigating claims on behalf of patients of Cook County Health whose information may be have stolen in a recent data breach involving a vendor (Perry Johnson & Associates, or PJ&A) in April 2023. Cook County Health is notifying patients that their personal information, including names, dates of birth, Social Securitt Numbers, addresses, medical record numbers, encounter numbers, medical information, and dates/times of service. may have been stolen as part of a recent hack.

If you have received a recent notice of the data breach, it is possible that your personal medical information was compromised and is being offered for sale on the dark web.
